Public Health Improvement Plan

In 2013, after several years of research and community assessments, Clear Creek Public and Environmental Health submitted a Public Health Improvement Plan (The Plan) to the state as mandated by the Colorado Department of Public and Environmental Health.  The Plan outlined several health concerns of the county with a list of goals and action items to address three priority issues:  Health Care Access and Availability, Transportation Access and Coordination, and Mental Health/Substance Abuse Prevention and Treatment.

In 2014 a series of convenings were held to engage the community in action items and solutions that address the priority issues
of the county as listed in The Plan.  Evaluations and surveys were implemented at each convening, with summary results available in visual charts.  Work has progressed in 2014 to include stakeholder meetings, grant applications and a request for proposals (RFPs).

Minutes and notes of the meetings as well as upcoming activities are available on this Web site for the general public and those interested and involved in the work of The Plan.  A contact data base has been compiled of community members and stakeholders interested in progress towards the goals of The Plan.
