Public Health Improvement Plan

In 2013 Clear Creek County Public and Environmental Health (CCCPEH) submitted our first Public Health Improvement Plan to the state as mandated by the Public Health Act of 2008. This document outlined health concerns for the county with goals and strategies to address priority issues: Health Care Access and Availability, Transportation Access, and Mental Health/Substance Abuse.

Colorado Department of Public and Environmental Health (CDPHE) through the Office of Planning, Partnerships and Improvement supports the Colorado Health Assessment and Planning System (CHAPS) with funding and technical assistance. More recently in 2016, our local health department has begun working into the new 5-year cycle for the CHAPS process. This began in the spring as a partnership with graduate students from the Colorado School of Public Health. The student group was from the Community Health Assessment (CHA) class and they completed data analysis and community focus groups. The results from this project are described in the following report: Clear Creek County 2016 Preliminary Community Health Assessment

The health department will continue with additional activities to complete an updated CHA to inform the next Community Health Improvement Plan (CHIP) that will be published in 2018.
