Health First Colorado (Medicaid)

Health First Colorado (Medicaid) is public health insurance for Colorado residents who qualify based on family size and income. Individuals, families, children, pregnant women, and persons who are blind, disabled or elderly may qualify. Individuals who receive Medicare may also qualify for Health First Colorado (Medicaid) to assist with Medicare premiums.

Colorado Health Plan Plus (CHP+)

CHP+ is a low-cost health insurance program for Colorado children under the age of 19 and pregnant women whose families income is over the limit for Health First Colorado (Medicaid) but less than 260% of the federal poverty level. Your family size, income and age of the applicant will be used to determine your eligibility for Health First Colorado (Medicaid) or CHP+.

If I know I am not eligible for Health First Colorado (Medicaid)/CHP+ why must I apply for these programs to obtain tax credits through Connect for Health Colorado?

If you want to receive tax credits toward the cost of health insurance through Connect for Health Colorado you must first be denied for Health First Colorado (Medicaid)/CHP+ eligibility. If you are found eligible for Health First Colorado (Medicaid)/CHP+ you will not be eligible for tax credits through Connect for Health Colorado.

Applying

English and Spanish Applications are also available at Clear Creek County Human Services. We are located in the basement of 405 Argentine Street in Georgetown, and in the Health and Wellness Center at 1969 Miner Street, Idaho Springs.

The following documents may expedite the processing of your application:

  • Proof of Citizenship (Birth Certificate, Passport, Permanent Resident Card, Certificate of Naturalization, etc.)
  • Social Security numbers for all household members
  • For individuals who are self employed: proof of income such as tax returns or profit/loss statements
  • Verification of income from other sources (rental income, VA, pension, unemployment, etc.)
  • For undocumented individuals proof of earned income

Application-Processing Timeline

Health First Colorado (Medicaid) applications will be processed within 45 days from the date our office receives your completed application and all required documents.
